When i attempt to go to the contact page on my site I get the following error

Let me give you some background. I am having this issue on my test site which is on my local server. When I originally purchased and installed your package all was well. My production site is also working correctly. That is I am able to present the attached page but not send. When I click on the send comments button I received the attached.

In my attempt to investigate why I was getting this error in prod, I noticed that the version of the package in prod was newer than the one I have on my test site. I proceeded at that point to install the more current version on my test site. After doing so I began to get the error I stated in my previous email. I am not getting that error in prod only on my test site. I hope this provides more info so you can help me resolve the issue.

furthermore, In doing some further research I think the issue is with the fields, see attached. The field ids that I have in the attached do not exist. The page displays properly without any fields, i.e., no error. If I place real field ids in the above error occurs.
